Disclosure Veto

This form applies only to adoptions granted prior to January 1, 2005. A written veto may be filed by:
  • an adoptee, 18 years of age or older
  • a parent (biological parent or adoptive parent under a previous adoption order).

A veto prohibits the release of your identifying information from the sealed adoption file.

A veto may be cancelled in writing at any time.

A veto is no longer in effect when the person who filed the veto is deceased.

A photocopy of two pieces of identification is requested along with this form. One piece must be photo identification.

A person filing a veto may include a written or medical statement for release to the other party.

The Post Adoption Registry will provide any written or medical statement that is filed with a veto, when the adoptee or birth parent requests information.
